Course Participant Prescreen Questionnaire
If you would like to participate in a Mindfulness SWO MBSR course, you must complete the prescreen questionnaire below and then obtain a doctor’s referral. Please see below for full enrolment requirements.
How to Register for Our Courses
After completing the 3-question prescreen questionnaire (below), you will be given the referral form for your doctor if you qualify based on your responses. If you do not qualify, you can contact us to discuss whether MBSR is a good fit for you. After receiving your doctor’s referral, we will contact you to confirm your registration and book an intake appointment. (You are not registered until your doctor’s referral is received.)
